All Food Service Establishments are required to Register by June 30 of each calendar year for the period July 1 through June 30. There is no cost to register if the registration is submitted prior to facility start-up and then annually thereafter by June 30. For ongoing operations, failure to register by June 30 will require payment of a $75 registration fee.
Registration may be accomplished by completing the online registration form or by completing the FOG Registration Form in Microsoft Word or Adobe Acrobat and submitting the form by email, fax, or mail. The City is not responsible for lost or redirected mail. For registration forms received by mail, the USPS date stamp shall serve as the date of registration.
